What you've just described is exactly what VSI, one of the pioneering producers used to do. Unfortunately they also included the 15 minutes worth of practicing with unlit cigs in their final product, which was very frustrating to watch, let me tell you! When the model did finally get to smoke the result was sometimes quite good, and sometimes not.

In my experience, and I have quite a reasonable collection of videos (over 60 titles), smoking fetish models can't act. By far the best smoking fetish videos are ones in which the model is not asked to play a scene, but rather just smokes or better still interacts in a relaxed way with other models or with the producer. Some producers are very good at this and often the ones who are in the modelling business and who use professional models (e.g. Smoke Vision) are the best in my view.

I'm not old enough to remember commercials from the 50's and 60's and even if I was, I wouldn't because we didn't get TV in my country till the 60's and even then never had cigarette ads. I have downloaded a few low quality MPEGs of them from websites and seen a few in documentaries but although they were interesting, I can't say they did that much for me. Given the choice I'd take a good Smoke Vision video any day.
